When I receive this Personalization Form, I will personally email you to verify your order and send you a link to my Venmo or Paypal for payment. I will create your personalized moon when I receive payment. If your order is being shipped, I will need the address and recipient’s name. Shipping is free if inside the U.S. International shipping requires an additional charge.

Once I have started making your order, cancellations are not accepted. I can’t wait to make the moon for you!
